In the days before unions and strict on-set regulation, filmmakers could get away with something like this. Hundreds of extras were positioned in a water tank, only to have 15,000 tons of water poured over them to simulate the famous flood. Three people died as a result.

The Wizard Of Oz (1939)


Mostly the stuff of urban legend, no one DIED as a result of making this movie. Actor Buddy Ebsen did have to leave the production, however. He was originally cast as the Tin Man, but the make-up used contained real aluminum. An allergic reaction sent him to the hospital and a new actor had to take over.
